Put "Strick" into the search box. Therapy Appointment software will then guide you through the registration process as a new client/patient. Please complete the biographical form that is included with the registration, and upload a copy of both sides of your insurance card in the insurance section, and in the profile section upload a copy of both sides of your driver's license as a form of ID.
Treatment consent forms will be emailed to you. Please complete, scan, and then email back to me prior to your first appointment. Thank you.
